Spoon River Home Health is looking for a compassionate and dependable Registered Nurse (RN) to join our team on a PRN basis, providing skilled nursing visits to patients in Peoria and Tazewell counties.

This is a hands-on, field-based home health position for an RN who enjoys working independently and providing one-on-one care in patients' homes. We are specifically seeking an RN with Monday and Friday availability.

Patient visits are compensated on a per-visit basis, with rates varying based on the type of visit performed. Applicable non-visit work, such as required nursing meetings, is compensated at an established hourly rate within a $35-45 range.

• Travel to patients' homes throughout Peoria and Tazewell counties
• Provide skilled nursing care based on patient needs and physician orders
• Complete patient assessments and ongoing evaluations
• Perform skilled nursing visits and required clinical documentation
• Monitor changes in patient condition and communicate with physicians and the care team
• Educate patients and families regarding medications, treatments, diagnoses, and care needs
• Collaborate with supervisors, therapists, physicians, and other members of the interdisciplinary care team
• Follow agency policies, clinical standards, and regulatory requirements

• Current Illinois RN license
• Minimum of 1 year of nursing experience
• Availability on Mondays and Fridays
• Strong clinical assessment and critical-thinking skills
• Excellent time management and organizational skills
• Comfortable working independently and traveling to patients' homes
• Dependable, self-motivated, and compassionate
• Strong communication and documentation skills
• Home health experience preferred, but not required
