Admissions Coordinator

A Cassena care facility located in the Bronx is actively seeking an Admissions Coordinator to join our team of dynamic professionals!

• Facilitate admissions from hospital and community setting into the facility
• Review referral packets from hospital with nursing department to determine if clinical needs can be met
• Facilitate ongoing communication with referral sources including but not limited to hospitals, assisted living facilities, community agencies and family members
• Provide information and tours of the facility - either in person or virtual - in order to best assist family members through the admission process
• Prepare both physical chart for nursing unit and complete electronic medical record
• Remain available and responsive to hospital social workers to assist with setting up admissions to the facility
• Review case and obtain authorization from managed care companies
• Complete financial screenings to ensure insurance coverage for the facility
• Remain available to patients and family members during the course of their stay to ensure satisfaction
• Work with interdisciplinary team 
• Participate in facility events 

Qualifications:

• Minimum Associates Degree preferred
• Minimum 2-5 years’ experience as a successful Admissions Coordinator in a short term rehabilitation/long term care facility preferred
• Healthcare, sales, marketing experience preferred
• Knowledge of Microsoft Office, typing, medical terminology knowledge and data entry required
• Excellent interpersonal skills; possess the ability to deal tactfully with residents, family members, visitors.

Bi-lingual English/Russian preferred
