Support Coordinator - FT Home 6 - 28hrs/wk

POSITION SUMMARY
This is a front-line disability services role that supports the development of skills and relationships that aid persons with disabilities work towards and meet their goals. This position will support individuals with a wide range of abilities and support requirements in both their home, and their community.
PREFERRED QUALIFICATIONS
- A positive attitude that acknowledges the gifts and passions of persons with disabilities, an aptitude for service and support, and a relevant knowledge and skill base.
- Secondary school education in a relevant discipline, from a recognized institution with additional specialized training and/or demonstrated successful experience in a relevant setting as defined by the employer based on individual needs and services required.
- First aid and CPR for adults is required.
- Candidates with a class 5 driver's license, personal vehicle, clear drivers abstract, and 2-million-dollar liability insurance will be given preference
- Willingness to complete a Police Information Check with the Vulnerable Sector Search is required
- Candidates who have experience in coaching emotional regulation will be given preference
RESPONSIBILITIES
- Support and implement person centered plans for personal development, community inclusion, behavior management, and personal care. Assist individuals to set and achieve personal goals.
- Provide meaningful interactions and the appropriate level of support to allow the individual maximum participation in their day-to-day life, promoting independence and self-determination.
- Help individuals responsibly manage finances, including handling money for activities and maintaining accurate records.
- Follow all practices, procedures and protocols to ensure the health and safety of the individuals we support, coworkers, visitors and the work environment.
- Report incidents and minimize risks while promoting participation
- Support healthy eating practices, medical requirements, use and maintenance of environmental interventions/ adaptive technology and personal care.
- Participate in worksite cleaning and maintenance
- Support individuals in building relationships, advocating for themselves and participating in employment and/or social activities. Work with and develop natural support systems to enhance inclusion.
- Actively participate as a team member- share information, seek assistance when needed, and follow direction
- Stay informed about and follow policies and procedures, maintain ethical standards, ensure required training is completed and complete accurate documentation.
- Documentation requirements including incident reporting, medication administration, contact notes, log notes, communication book entries, financial recording, inspection reports, etc.
- Always communicate (verbally and non-verbally) in a manner that is professional and respectful and support individuals’ communication needs.
- Knowledge of mental health crisis preferable.
- Strong rapport building skills and creative community engagement an asset.

HOURS OF WORK: Thu/Fri 3:00PM - 9:30PM
Sat/Sun 8:00AM - 3:30OM
