Reports to: Director of Nursing
Job Description: As an RN or LPN in a Skilled Nursing Facility, you will be responsible for providing skilled nursing care and oversight to residents with complex medical conditions, chronic illnesses, or disabilities. You will collaborate with other healthcare professionals, including physicians, therapists, and nursing assistants, to assess, plan, implement and evaluate care for residents.
Key Responsibilities:
- Assess residents' health status and develop and implement individualized care plans that address their unique needs and preferences.
- Administer medications and treatments as ordered by physicians and ensure residents' compliance with prescribed regimens.
- Monitor and document residents' vital signs, symptoms, and response to treatments.
- Collaborate with interdisciplinary teams to manage residents' medical needs, including wound care, physical therapy, and other therapies.
- Coordinate with families and community resources to promote residents' physical, emotional, and spiritual well-being.
- Participate in quality improvement programs, infection control, and risk management initiatives.
- Provide support and guidance to nursing assistants and other staff members, as needed.
- Perform other duties as assigned.
Qualifications:
- Active Registered Nurse (RN) license or Licensed Practical Nurse in the state of Ohio
- Knowledge of nursing principles, practices, and techniques, as well as current trends and developments in the field.
- Ability to assess and interpret data and communicate effectively with residents, families, and other healthcare providers.
- Strong interpersonal skills and the ability to work collaboratively with interdisciplinary teams.
- Basic computer skills to maintain electronic medical records and documentation.
- Ability to work flexible schedules, including weekends and holidays.
Physical Demands:
The RN or LPN in a skilled nursing facility position requires the ability to perform essential functions of the job, including the ability to stand, sit, walk, bend, lift, and move intermittently throughout the workday. The position also requires the ability to communicate clearly and effectively with residents, families, and healthcare providers.
