Reports to: Director of Nursing

Job Summary: The Licensed Practical Nurse (LPN) provides basic medical and nursing care to patients within a skilled nursing facility. The LPN works under the supervision of a registered nurse or physician and provides direct patient care such as medication administration, wound care, monitoring vital signs, and assisting with activities of daily living.

Key Responsibilities:

Administer medications and treatments as prescribed by the physician or registered nurse

Monitor and record vital signs, such as blood pressure, temperature, pulse, and respiration

Assist with the activities of daily living, including bathing, dressing, grooming, and toileting

Perform basic nursing procedures, such as catheterization, tube feedings, and wound care

Document patient care activities in the medical record accurately and in a timely manner

Communicate with physicians and other healthcare team members to ensure continuity of care

Participate in interdisciplinary team meetings to discuss patient care and treatment plans

Adhere to facility policies and procedures, including infection control, safety, and HIPAA regulations

Maintain a clean and organized work environment

Provide emotional support and education to patients and families

Qualifications:

Graduate of an accredited practical nursing program

Active LPN license in the state of employment

Current CPR certification

Minimum of 1-year experience in a long-term care or skilled nursing facility preferred

Excellent communication and interpersonal skills

Ability to work independently and as part of a team

Strong organizational and time-management skills

Compassionate and empathetic towards patients and families

Ability to lift and move patients as needed

Physical Demands:

The LPN must be able to lift and move patients (up to 50 pounds), as well as stand, walk, and bend for extended periods of time. The LPN must have visual acuity and manual dexterity to perform nursing procedures accurately and safely.

Work Environment:

The LPN works in a healthcare setting that may involve exposure to infectious diseases, unpleasant odors, and verbal or physical aggression from patients or family members. The LPN may also be required to work evening, night, and weekend shifts as needed.
