Wound Care Nurse (LPN or RN)
Position Summary:
We
are seeking a dedicated and skilled Wound Care Nurse (LPN or RN) to join
our healthcare team. The ideal candidate will be responsible for
assessing and managing patients with wounds, ensuring optimal healing
through evidence-based practices. This role requires strong clinical
judgment, compassionate patient care, and the ability to collaborate
effectively with a multidisciplinary team.
Key Responsibilities:
- Conduct comprehensive assessments of patients with acute and chronic wounds.
- Develop and implement individualized care plans based on thorough evaluations.
- Perform wound care treatments, including dressing changes, debridement, and infection prevention.
- Educate patients and families on wound care techniques and prevention strategies.
- Collaborate with healthcare professionals to coordinate and enhance patient care.
- Document assessments, interventions, and patient outcomes accurately and in a timely manner.
- Participate in quality improvement initiatives related to wound care management.
- Stay updated on the latest advancements in wound care practices.
Qualifications:
- Education:
- Current valid license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
- Experience:
- Previous experience in wound care or related nursing specialty preferred.
- Certifications:
- Certification in wound care (e.g., CWCN, CWS, or equivalent) is a plus.
Skills:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Proficient in educating patients and families.
- Knowledge of wound care products, treatments, and infection control.
- Familiarity with electronic health record (EHR) systems.
Work Environment:
The
Wound Care Nurse may work in various settings, including hospitals,
outpatient clinics, long-term care facilities, or home health
environments. Flexibility in scheduling is required.
