Registered Nurse (RN) – Hospice - $5000 Bonus

Role Snapshot (What This Job Actually Looks Like)
Caseload: 18–25 patients, structured and supported across the team
Daily Visits: Typically 5–6 visits per day, organized by territory
Territory: Focused coverage in North and Northeast Philadelphia (no multi-county travel)
Schedule: Field-based with structure hours Monday-Friday 9:00 AM-5:00 PM
On-Call: Shared, structured rotation across the team (limited and evenly distributed)
System: Kinnser (WellSky) for efficient documentation and compliance
About the Role
This is a field-based hospice RN position for nurses who are confident working independently, managing patient care, and making clinical decisions in a home setting. You will be responsible for delivering high-quality end-of-life care while maintaining a structured, manageable caseload and consistent territory. This role is best suited for RNs who value autonomy, stability, and a well-organized clinical environment.
Key Responsibilities
Conduct initial and ongoing patient assessments
Develop and manage individualized care plans
Provide skilled nursing care, including pain and symptom management
Educate and support patients and families through end-of-life care
Maintain accurate, timely documentation using Kinnser
Collaborate with the interdisciplinary team to ensure quality care delivery
What We’re Looking For
Active Pennsylvania RN license (required)
Hospice experience strongly preferred; strong clinical background considered
Ability to manage a caseload independently with sound clinical judgment
Comfortable working in a field-based environment
Reliable transportation and willingness to travel within assigned territory
