Registered Nurse (RN) – Hospice

Position Overview:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our hospice care team. The ideal candidate is a strong communicator who thrives in an autonomous role and is passionate about providing holistic, patient-centered care.

Key Responsibilities:

• Conduct initial and ongoing patient assessments to determine physical, emotional, and spiritual needs.

• Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.

• Provide direct skilled nursing care, pain and symptom management, and emotional support to patients and families.

• Educate family members on end-of-life care, medications, and comfort measures.

• Document all visits accurately and timely in accordance with agency policies and regulatory standards.

• Participate in on-call rotation and interdisciplinary meetings.

Qualifications

• Active Registered Nurse (RN) license in Pennsylvania - required

• Minimum 2 year of professional hospice experience - strongly preferred

• Demonstrated ability to perform thorough clinical assessments, prioritize effectively, and manage complex caseloads with autonomy

• Exceptional communication, critical thinking, and documentation skills

• Valid driver’s license, reliable transportation, and willingness to travel to patient homes as needed

• Commitment to maintaining professional integrity, compassion, and high standards of patient care
