Hospice RN Case Manager
Position Overview: Unified Care Hospice is seeking a compassionate and dedicated Registered Nurse Case Manager to join our dynamic team. As a Registered Nurse Case Manager, you will play a pivotal role in providing holistic and patient-centered care to our hospice patients and their families. Responsibilities:
Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs, collaborating with the interdisciplinary team to develop personalized care plans.
Coordinate and manage patient care, ensuring that interventions are consistent with patients' wishes, goals, and the hospice philosophy.
Provide skilled nursing care, pain and symptom management, and emotional support to patients and their families in accordance with established protocols.
Collaborate closely with physicians, social workers, chaplains, and other team members to address the diverse needs of patients and families.
Communicate effectively with patients, families, and the care team, providing education about the hospice process and supporting informed decision-making.
Monitor patients' condition, update care plans as needed, and ensure timely documentation in electronic medical records.
Participate in regular interdisciplinary team meetings, contributing insights and expertise to enhance patient care delivery.
Serve as a resource for healthcare providers, patients, and families, offering guidance on pain management, symptom control, and end-of-life care.
Engage in continuous education and training to maintain and enhance your clinical skills and knowledge.
Actively participate in team events, fostering a collaborative and supportive work environment.

Adhere to ethical and professional standards, maintaining confidentiality and upholding the hospice's reputation for excellence.
Qualifications:
Current Registered Nurse (RN) license in the state of practice.
Hospice or Home Health experience preferred
Strong communication and interpersonal skills, with the ability to empathize and connect with patients and families.
Proficiency in electronic medical records and documentation.
Ability to work effectively within an interdisciplinary team.
Commitment to providing patient-centered and compassionate care.
Reliable transportation for patient visits and mileage reimbursement
Comfortable with travel and a flexible schedule.
