Registered Nurse

The RN will be responsible for completing In-home or Personal Care Home Skilled Nurse Visit as indicated.

• The In-home Skilled Nurse visit can include but not limited to: wound care, medication set up, lab draws, catheter care and catheter insertion, diabetic care and education, health education/teaching, observation and assessment, ostomy care, and prosthesis care.

• The Personal Care Home visit can include but not limited to: reviewing and signing member service records, careplans, activity logs, daily sign-in sheets, CCNFs, MIFs, MARs and progress notes monthly and Supervisory Visits bi-weekly
