"REGISTERED NURSE"
Position Summary:
The Registered Nurse (RN) – Home Health Assessment Nurse is responsible for conducting comprehensive in-home nursing assessments for new clients, resumption of care, and ongoing 60-day recertifications. The RN ensures accurate evaluation of patient needs, development of individualized plans of care, and compliance with state regulations, Medicare/Medicaid requirements, and agency policies.

Perform Start of Care (SOC) assessments for new home health clients

Conduct Resumption of Care (ROC) assessments following hospitalization or interruption of services

Complete 60-day recertification assessments to determine continued eligibility and service needs

Assess patient’s medical condition, functional status, ADLs, safety risks, and home environment

Review medications for accuracy, compliance, and potential interactions

Develop, update, and coordinate the Plan of Care (POC) in collaboration with physicians and interdisciplinary team

Identify skilled nursing needs and recommend appropriate home health services

Provide patient and caregiver education regarding disease management, medications, and safety

Ensure all assessments and documentation are completed accurately, timely, and in compliance with regulatory standards

Communicate changes in patient condition promptly to physicians and agency leadership

Maintain patient confidentiality and uphold ethical nursing standards

Active Registered Nurse (RN) license in good standing

Minimum of 1–2 years of nursing experience (home health experience preferred)

Knowledge of home health regulations, OASIS documentation, and care coordination

Strong assessment, documentation, and communication skills

Ability to work independently and manage multiple assessments efficiently

Valid driver’s license and reliable transportation
This position requires travel to patients’ homes and the ability to perform physical assessments in a home setting.
