Bring your clinical experience to a leadership role where you can guide a team and help adults living with serious mental illness build greater independence. At Community Hope, you’ll oversee the Community Support Services (CSS) program while supporting individualized care, wellness, and recovery.
SCHEDULE
- Monday through Friday: 8:00 AM–4:00 PM or 9:00 AM–5:00 PM
- On-site, with the option for one remote workday per week after training

YOUR ROLE
You’ll combine team leadership with direct clinical work, helping ensure services reflect each individual’s needs, choices, and personal goals.
- Lead and develop the CSS team through clinical supervision, training, coaching, and performance evaluations.
- Assess rehabilitation needs and oversee the development, implementation, and review of individualized rehabilitation plans.
- Provide and coordinate counseling, therapeutic skill development, referrals, and community-based wellness and recovery services.
- Provide crisis intervention and participate in the on-call rotation. When fully staffed, on-call coverage is approximately one week every six weeks, covering hours outside the regular 9:00 AM–5:00 PM or 8:00 AM-4:00 PM workday, Monday through Friday.
- Submit recommendations for care authorization and maintain accurate, confidential documentation in accordance with program, fiscal, and billing requirements.
- Contribute to program improvements, new initiatives, and community engagement.
Additional responsibilities include monitoring medication self-administration when needed and occasionally transporting individuals in agency vehicles—approximately twice per year.
QUALIFICATIONS
- Current New Jersey license as a Clinical Social Worker (LCSW), Professional Counselor (LPC), or Registered Nurse (RN)
- Knowledge of mental health rehabilitation principles
- Valid New Jersey driver’s license with a good driving record
