PRN Physical Therapist – Home Health

Location: Based in Youngstown, OH (Travel required)
Employment Type: PRN (As Needed)
Pay Range: $50 – $60 per hour (depending on experience)

We are seeking a licensed and motivated PRN Physical Therapist to join our growing home health team. This position is ideal for therapists who value autonomy, flexibility, and one-on-one patient care in a home setting. You will conduct evaluations and provide skilled physical therapy to patients in their homes throughout Youngstown and extended areas.

• Perform in-home physical therapy evaluations and establish individualized care plans

• Deliver skilled therapy to improve strength, mobility, function, and quality of life

• Educate patients and caregivers on therapy techniques and safety

• Accurately document all services provided in a timely manner using electronic medical records (EMR)

• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care

• Travel to patient homes within assigned service area, including flexibility to cover visits in Mahoning County

• Current Ohio Physical Therapist (PT) license in good standing

• Minimum of one (1) year of experience in a clinical or home health setting preferred

• Reliable transportation and willingness to travel extended distances as needed

• Valid driver’s license and auto insurance

• Strong documentation and time management skills

• Comfortable working independently in patients’ homes

• Experience with EMR systems (e.g., WellSky, Homecare Homebase, Kinnser)

• Familiarity with Medicare and OASIS documentation standards

• Prior home health or geriatric care experience
