Job Summary: Home Care Coordinator

Position Overview:

The Home Care Service Coordinator is responsible for providing coordinated care to patients by developing, monitoring, and evaluating their home health care plans. This role ensures quality care delivery, effective scheduling, and excellent communication with caregivers, clients, and referral sources. The ideal candidate thrives in a fast-paced environment, is highly organized, and committed to delivering outstanding customer service.

Key Responsibilities:
  • Develop, monitor, and evaluate patient home care plans.

  • Ensure high-quality care is delivered by home health aides.

  • Schedule agency home health aides and ensure shift coverage.

  • Manage last-minute call-outs and find appropriate replacements.

  • Build and maintain an on-call pool of aides in coordination with HR.

  • Communicate effectively with referral sources and case managers.

  • Listen to client needs and match them with suitable caregivers.

  • Manage a caseload of 100+ patients.

  • Review care plans with patients and caregivers.

  • Resolve grievances or complaints from caregivers and clients.

  • Monitor and address overutilization of service hours.

  • Report and document personnel performance issues.

  • Ensure retention of patients within your caseload.

  • Support team efforts and contribute to departmental goals.

Minimum Qualifications:
  • High school diploma or equivalent.

  • 2+ years of experience in home care or service coordination.

  • Ability to maintain confidentiality.

  • Proven track record in lead generation and referral tracking.

  • Proficient typing and computer skills.

  • Strong organizational skills with attention to detail.

  • Excellent communication and customer service skills.

  • Experience with HHA Exchange and EVV systems.

  • Bilingual in English/Spanish preferred.

  • Ability to ask for business and close effectively.

Compensation and Benefits:
  • Salary: $18.00 - $23.00 per hour

Schedule:
  • Full-Time

  • Monday to Friday

  • 8-hour shifts
