Position OverviewWe are seeking a dedicated and compassionate Occupational Therapist (OT) to provide in-home therapy services to patients across our California service areas. In this role, you will help patients regain independence, improve daily functioning, and enhance their quality of life through personalized care plans.
Your work will directly impact patients’ ability to live safely and confidently in their own homes.
Key Responsibilities-
Perform comprehensive occupational therapy evaluations in patients’ homes

Develop individualized treatment plans based on patient needs and physician orders

Provide therapeutic interventions to improve daily living and functional skills

Educate patients and caregivers on safe practices, adaptive techniques, and equipment use

Monitor patient progress and modify treatment plans as needed

Maintain accurate and timely clinical documentation in compliance with regulations

Communicate effectively with physicians, nurses, and interdisciplinary team members

Ensure compliance with Medicare, state, and agency policies

Participate in care coordination and case conferences as required

Current Occupational Therapist license in the State of California

Graduate of an accredited Occupational Therapy program
-
Minimum of one year of experience in home health preferred (new graduates may be considered)

Strong clinical assessment and treatment planning skills

Excellent communication and interpersonal abilities

Ability to work independently and manage caseload efficiently

Reliable transportation and valid driver’s license

Current CPR certification

Proficiency with electronic medical records (EMR) systems preferred
