Nurse Assessment Coordinator (RN or LPN)

Transitions Healthcare North Huntingdon is currently hiring a Nurse Assessment Coordinator. The individual for this position could be considered as either a RNAC or LPNAC primary purpose in this position is to work with the MDS Coordinator to plan and implement the MDS completion in accordance with facility procedures as well as State and Federal regulations.

Nurse Assessment Coordinator (LPNAC) or (RNAC) Position Highlights Include:

• Gathers necessary data for completion of the MDS.
• Assists in identifying significant changes in residents’ condition.
• Monitors resident care plans for accuracy and completion.
• Attends the Medicare meeting.
• Assists in education of the MDS process with nursing staff and the Interdisciplinary care team.
• Data entry of MDS information.
• Reviews messages from contracted pharmacy regarding medications needing prior authorization.

Nurse Assessment Coordinator (LPNAC) or (RNAC) Minimum Qualifications

• Current licensure as either a practical nurse or a registered nurse in the Commonwealth of Pennsylvania
• CPR/AED certification
• Minimum of one (1) year nursing experience
• Proficient in MS Office Suite and Outlook
