Essential Duties & Responsibilities: Coordinate preconceptional and interconceptional services for prenatal, non-pregnant high-risk women and their partners.
Specific Activities:
Case Management
· Coordinate prenatal, pre- and interconception services for high-risk women.
· Utilize selected standardized curriculum and intervention and document activities.
· Assess risk and develop risk reduction goals/activities and link participant to services.
· Ensure that a comprehensive Life Course Plan is developed for each participant
· Link male partners of enrolled women to group activities and individual services.
· Assist with and ensure participation in scheduled the Life course group activities.
· Provide services in participant’s home, or current living location.
· Provide social and mental health linkages and support.
· Follow up with all providers involved with the family.
· Provide feedback to all agencies involved.
· Participant in case conference as required.
· Complete and maintain all documentation, ensuring confidentiality
Other:
· All other duties as assigned by the Team Lead or Magnolia Project Director.
Education and/or Work Experience Requirements:
· Bachelor degree in the field of maternal child health or social science preferred.
· Will substitute 5 years professional experience in direct service, social services, maternal and child health or related field.
· Experience working with diverse communities, organizations and agencies preferred.
Additional Eligibility Qualifications:
· Knowledge of local health and human service providers
· Ability to work flexible traditional and non-traditional hours
· Ability to be a self starter and work independently as necessary
· Good communication oral and written
· Maintain professional boundaries
· Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines, and standard accepted practices.
· Must be able to communicate effectively and be a team player at the Coalition.
· Must be able to commit to the Coalition vision and be passionate about its mission.
· Requires reliable transportation as travel is required, valid driver’s license, good driving record and automobile insurance.
Physical Requirements:
· Ability to safely and successfully perform the essential job functions consistent with the ADA, and other federal, state and local standards.
· Must be able to travel to alternate locations.
· Required to sit, stand, walk, speak and hear while performing the duties of this job, regularly. The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen
· Lifting/carrying up to 25 pounds.
