The LPN Wellness Coordinator will be responsible for all nursing-related components of the admission process. This includes, but is not limited to, conducting nursing assessments, verifying and transcribing admission orders, and ensuring medications arrive in a timely manner.
LPN Wellness Coordinator Job Highlights:
Daily Operation Responsibility: Audits med admin, glucose monitoring, weights/vitals. Document care assists with MD orders, Forms that need to be filled out, Bi-monthly recaps on Tuesdays. Chart audits, Rasps updated from orders, be the other face of personal care every day to families and residents. Follow up with clarification orders, pharmacy issues. Help on the floor when needed answering bells, toileting, doing ADLS, passing meals, anything needed from PCAs or MT/LPN. Following up with residents in the hospital. Thinning charts, medical records.
Staff Management: Helping to track call offs, Supervise LPNs, MTs, PCAs. Following through with progressive disciplines along with auditing when tasks are not completed by staff. Keeping up with duties lists for PCAS (showers, vitals, weights, resident care). Assisting with staff educations and meetings. Following on call schedule and filling in for call offs for all disciplines.
Budget : Understand the concept of the budget and assist the PCHA with maintaining budget with helping to get shifts covered in house and cover shifts as needed. Help monitor medications and billing for residents we are responsible for. Practices fiscal responsibility of supplies, equipment, and time management.
Compliance: DME. RASPS, POLSTS, outside agencies paperwork signed by residents/ designated person. Racking monthly labs and getting them ready. Annual Tb screening and fall risk assessment. Annual flu/vaccine clinics for residents. Annual addendum updates for fees. Update LOC as needed for residents. Assist with chart audits. Assist with preparing and participating in DHS surveys.
LPN Wellness Coordinator Minimum Qualifications:
- Graduation from an accredited school of nursing as a Licensed Practical Nurse or Registered Nurse.
- Current LPN or RN licensure by PA State Board of Nursing or Multi State Licensure
- CPR certification (prior to or upon employment)
