The LPN (Licensed Practical Nurse) is responsible for ensuring the general well-being of Patients through the delivery of high quality services in a manner that is consistent with the philosophy of True Care Home Care Agency and that complies with applicable state laws and regulations. The LPN assists older people who are functionally, physically and/or socially impaired and need varying levels of oversight. The LPN’s role is to assist patients with those activities of daily living that they are unable to perform without help, fostering at all times patients’ independence and freedom of choice. He/she is responsible for assisting in the management and supervision of the HHA’s and other staff. Such supervision must be in accordance with current federal, state and local standards, guidelines and regulations that govern our facility and as may be required by the NM to ensure that the highest degree of quality care is maintained at all times.

What You Will Do:
  • Performing nursing duties under the supervision of RN; making visits as needed, preparing and administering medications, pre-pouring medications, taking vital signs, reporting and documenting information accurately and promptly.
  • Participating in programming and ROM activities.
  • Assists NM in orientations, on-going training, supervising, motivating & evaluating HHA’s and/or other staff to ensure provision of high quality services to Residents in a manner that is consistent with philosophy of True Care.
  • Provide assistance to HHA’s as needed which may include dressing & undressing (clothing should be clean & appropriate for the season), bathing, personal hygiene (shaving, dental hygiene, nail care, foot care, etc), mobility devices (cane, walker, wheelchair, crutches)
  • Works collaboratively with peers and other team members.
  • Following up with appropriate staff, or other individuals regarding reported complaints, problems and concerns.
  • Provides administrative assistance, such as answering phones, mail, copying, faxing, etc as needed.
  • Attend and participates in continuing educational programs.
  • Performs other related duties as assigned by NM.

Qualifications:
  • Graduate of an accredited school of practical nursing
  • Currently licensed in the State of NY
  • Experience in the care of elderly and dementia population is preferred, but not required
  • Must be organized; flexible in schedule; adaptable with great interpersonal skills to work well with the clients and families
  • Ability to operate appropriate equipment, e.g., wheelchair, walker, Hoyer lift, transfer boards, etc.
  • Must be able to lift, walk, carry, stand, bend, squat, crawl, reach and kneel using good body mechanics for sustained periods of time
