Home Health Registered Nurse ( RN)

Castor Home Health Solutions is looking for a compassionate and dependable Registered Nurse (RN) to join our growing home health team. This is a great opportunity for an RN who values independence, meaningful patient relationships, flexible scheduling, and the ability to provide personalized care outside of a traditional facility setting.

As a Home Health RN, you will work closely with patients, families, physicians, therapists, and other members of our care team to help patients recover safely and remain independent in their homes.

What You’ll Do

• Provide skilled nursing care in patients’ homes based on individualized plans of care.
• Complete comprehensive assessments and monitor patients’ conditions and progress.
• Perform wound care, IV therapy, catheter care, feeding tube care, laboratory specimen collection, and other ordered treatments.
• Complete OASIS assessments, as applicable.
• Educate patients and caregivers on medications, treatments, disease management, and home safety.
• Identify and promptly communicate changes in patient condition.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Complete accurate and timely electronic documentation.
• Provide compassionate, patient-centered care while promoting safety, independence, and recovery at home.

What We’re Looking For

• Active Illinois Registered Nurse license.
• Home health and OASIS experience preferred, but training and support may be provided.
• Strong clinical assessment and critical-thinking skills.
• Experience with wound care, IVs, catheters, feeding tubes, laboratory collection, and patient triage preferred.
• Excellent communication, organization, and time-management skills.
• Dependable, compassionate, and comfortable working independently in the field.
• Valid driver’s license and ability to travel to patients’ homes.
