Brockton Home Health Agency is a Medicaid and Medicare certified in Massachusetts and accredited by the Joint Commission - Accreditation of Healthcare Organizations (JCAHO) and we are currently working on getting another accreditation from CARF so we can extend our services through AFC and GAFC and more. Brockton Home Health Care agency is looking for a bi-lingual Community Health worker combine with an office manager to perform a variety of administrative and clerical tasks. Duties of the Administrative Assistant include providing support to assisting in daily office needs, managing our company’s general administrative activities also Conduct home visits to assess living conditions and provide guidance on maintaining a healthy environment

 

 Community Health Worker

·        Conduct outreach and engage with Adult Foster Care and Group Adult Foster Care members to assess their health needs and provide appropriate resources and support

·        Oversee all services provided to individuals and their families, monitor the quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.

·        Assist in the development of quality services and engaging activities that meet the individual served needs.

·        Build and maintain relationships with families and external case managers.

·        Provide health education and promote preventive measures to improve overall community health

·        Assist individuals in navigating the healthcare system and accessing necessary services

·        Conduct home visits to assess living conditions and provide guidance on maintaining a healthy environment

·        Collect data and maintain accurate records of client interactions and interventions

·        Collaborate with community organizations to coordinate health-related events and programs


 Office Manager

 ·        The Office Manager will be responsible for providing oversight and conducting a variety of office tasks, reviewing paperwork, and managing schedules

·        The Office Manager will also be responsible for billing, setup or remind clients about their annual physical with their PCP  and day-to-day operations

·        Maintains office services by organizing office operations and procedures; preparing payroll; controlling correspondence; designing filing systems; reviewing and

·        Maintains office efficiency by planning and implementing office systems, layouts, and equipment procurement

·        Designs and implements office policies by establishing standards and procedures; measuring results against standards; making necessary adjustments

·        Keeps management informed by reviewing and analyzing special reports; summarizing information; identifying trends

·        Experience:

·         work or related field is desirable

 

Qualifications

•          Two years of office work experience

•          One year of experience in health care setting

•          Must be familiar with Adult Foster Care (AFC) and/or Social worker

•          Demonstrated leadership ability

•          Excellent verbal and written communication skills

•          Commitment to quality care, and the multidisciplinary team approach
