West Janisch Health Care Center is seeking a confident and compassionate LVN or RN Charge Nurse to lead our team. We are currently hiring for both Full-Time and PRN positions, offering flexibility while maintaining strong clinical leadership on our day shift.

This role is ideal for a nurse who thrives in a fast-paced, skilled nursing environment and is comfortable leading a team while delivering high-quality, resident-centered care.

The Charge Nurse is a vital part of daily operations — serving as a clinical leader, mentor, and advocate for residents. Your leadership sets the tone for the shift and ensures smooth communication, strong teamwork, and excellent care delivery.

Key Responsibilities

• Provide direct nursing care in accordance with individualized care plans

• Supervise and support CNAs and nursing staff during the shift

• Administer medications and treatments as ordered

• Perform assessments and monitor changes in resident condition

• Communicate significant changes to physicians and leadership

• Ensure accurate, timely EMR documentation

• Maintain compliance with facility policies and regulatory standards

• Promote a safe, supportive, team-oriented environment

• Perform other duties as assigned

Qualifications

• Active Texas LVN or RN license in good standing (required)

• Skilled Nursing Facility (SNF) experience preferred

• Prior charge nurse or leadership experience strongly preferred

• Strong clinical judgment and organizational skills

• Effective communication and delegation abilities

• Compassionate, dependable, and resident-focused
