APPLICANT MUST HOLD CURRENT LOUISIANA CERTIFICATION AS A MEDICAL ASSISTANT.

JOB SUMMARY:

Affect the productive and efficient operation of assigned clinic or department through timely and attentive administrative support.   

  1. Ensure the safe, productive, and efficient operation of the clinic assigned through timely and attentive administrative and clinical support (as authorized or licensed).
  2. Ensure all patients enjoy a positive experience, treated with the care and compassion expected.
  3. Ensure all patient records and related documents are managed and maintained timely, accurately, and consistent with all HIPAA and related regulations and requirements.
  4. Ensure all tasks provided and associated with patient care, patient administrative processes, and related duties complies with all regulatory and accreditation standards, as well as clinic policies and procedures.  
  5. Develop favorable relationships with all patients. Instruct patients and their family members training and/or information related to in-home care as appropriate. 
  6. Establish favorable working relationships with all staff members associated with center operations, including Patient Access Clerks, nurses, physicians and related staff. 
  7. Uphold and consistently represent the values and mission of the organization at all times. Represent the organization in a highly professional manner at all times. 
  8. Ensure compliance and attention to all corporate policies and procedures.


Specific Duties and Responsibilities:

  • Prepares patient’s chart prior to clinical exam.
  • Prepares patient for clinical exam/visit, including the collection of typical patient vital metrics (blood pressure, temperature, height, weight etc.).
  • Assists provider with clinical visit.
  • Follows through with post-visit duties.
  • Backup the Patient Access Clerk positions and provide relief and support to Patient Access Clerk functions as requested.
  • Assists with office duties as assigned.
  • Provides service in a manner that is appropriate for the patient’s age; demonstrates knowledge and skills necessary to meet the patient’s physical, psychosocial, educational and safety needs.
  • Implement standards of Patient-Centered Medical Home (PCMH) through team-based care, managing patient populations, and supporting patients and families in self-management, self-efficacy, and behavior changes as needed
  • Participate in a structured communication process that is focused on individual patient care: discuss upcoming appointments, patient needs and workflow updates.
  • Collect and Document the following in the Certified Electronic Health Record Technology (CEHRT) system: patient and family medical history; patient and family behavioral health history; family/household structure; physical activity, alcohol consumption and smoking status; exposure to second hand smoke; dental care.
  • Administer a standardized tool for periodic developmental screening – newborns – 30 months
  • Denver Developmental Screening Test for Children – Ages 0-5 years
  • GAPS - Ages 6-11
  • Bright Futures Developmental, Behavioral and Psychosocial Screening – Ages 12 and up
  • Screen patients for depression using a standardized tool – Ages 12 and older
  • Community Health Centers: PHQ2 and PHQ9 for patients ages 12 and older
  • School Based Health Centers: Bright Futures, Developmental Behavioral and Psychosocial
  • Screening and RAAPS as needed– Ages 12 and UP
  • Review medications with patient at every visit to maintain an up-to-date list in the CEHRT.
  • Utilize the Patient Visit Communication Form and interview patients to determine preventative screenings that are due and document request for results if screenings were performed outside the clinic in the CEHRT system.
  • Provide patients with educational resources regarding tobacco cessation and nicotine use that are generated by the CEHRT; and distribute educational resources related to healthy eating.
  • Provide Diabetic and Hypertension patients with a self-management tool to track daily blood pressure readings.
  • Assist Diabetic or Hypertension Patients in documenting self-management goals with the use of a checklist.
  • Provide timely clinical advice by telephone and document in the CEHRT.
  • Submit electronic data to LINKS immunization registry to share immunization services provided to patients.
  • Assist patients with navigating the health care system.
  • Perform other assigned duties related to PCMH, such as:
  • Managing electronic patient requests for prescription refills; appointments to request a referral; and test results.
  • Use CEHRT system to track lab and imaging orders until results are available, flagging and following up on overdue results.
  • Flag abnormal lab and imaging results, bringing them to the attention of the clinician.
  • Notify patients of normal and abnormal lab and imaging test results per instructions documented by the ordering provider in the CEHRT system.
  • Promote the concept of Patient Centered Medical Home to patients, their families and their friends.
