Medicaid Coordinator

Hamilton Park Center for Nursing and Rehabilitation is looking for a Medicaid Coordinator to join the team at our Skilled Nursing Facility in Brooklyn, NY!

Medicaid Coordinator Responsibilities

●        Screen, interview, and assess each resident to determine Medicaid benefit eligibility

●        Receive and review admission forms for residents referred for Medicaid assistance

●        Collect all necessary documents to apply for Medicaid

●        Upload documents to be sent electronically to the Medicaid office

●        Correspond with HRA case workers to get the status of Medicaid applications

●        Follow up to ensure that all applications are approved

●        Obtain necessary information to determine eligibility for financial aid

●        Secure and complete appropriate forms in accordance with the established procedure

●        Perform other job-related duties as directed by Supervisor

Medicaid Coordinator Requirements

●        High school diploma or its equivalent

●        COVID-19 Vaccination

Preferred

●        Bachelor’s Degree in Business, Accounting/Finance, Marketing, Social Work, or a related field

●        Previous experience in a long-term care facility or another healthcare-related facility
