The LPN at the Mustard Seed Community Hub is committed to helping people who are experiencing homelessness and/or are in a vulnerable transition toward maintaining sustainable and healthy lifestyles. They work as an integral part of the broader Community Hub team to provide holistic services to community participants. The ideal candidate is a strong team player, with exceptional compassion and engagement skills. This candidate must hold a valid, current LPN license, and have the ability to be flexible, prioritize tasks, and work in a busy environment. Furthermore, this candidate must have a desire to serve our community participants, support staff and build community.

The candidate will report to the Community Development Manager, and in partnership with a neighbouring agency, will also maintain accountability to an appointed Physician or Registered Nurse.

DUTIES AND RESPONSIBILITIES

  • Execute initial triage, and meet with community participants who visit the Community Hub
  • Complete initial assessments to determine immediate interventions, and appropriate follow up supports and referrals
  • Provide basic routine patient care, observation of community participants health and engage community participants in treatment options and planning
  • Provide foot care to community participants
  • Respond to medical emergencies, including overdoses within the centre
  • Educate community participants on safe usage and harm reduction practices
  • Educate and empower community participants to understand their patient rights, and foster their well-being
  • Build a comprehensive library and knowledge base on community resources
  • Liaise, consult and advocate with internal and external resources that will help community participants nurture their strengths and resources
  • Work as part of a greater team within in the Community Hub to identify and meet varying needs of community participants
  • Provide input into the development of policies and procedures for designated programs
  • Provide input into program evaluation upon request
  • Document community participants interactions, services, referrals and progress
  • Keep community participants records and information confidential, updated and secure
  • Track statistics and prepare reports, as required
  • Attend meetings, and training, as assigned
  • Other duties as assigned

MINIMUM QUALIFICATIONS AND SKILLS

  • Valid, current LPN or equivalent
  • Minimum of 1-2 years of experience
  • Enjoy working in a fast-paced environment, engaging in quick intervention, and problem-solving
  • Strong competency and experience using data management systems, software programs and MS Office
  • Excellent administrative, time management, and organizational skills
  • Excellent written and verbal communication skills
  • Self-motivated and able to work independently and with multiple teams
  • Previous experience working with a marginalized population, particularly the homeless is considered an asset

WORKING CONDITIONS

  • Part time position (0.7 FTE). Time in excess of regular working hours may be required
  • Variation of hours/working days as required
  • Local travel to various Mustard Seed sites and external agencies may be required

  • Must have legal authorization to work in Canada on a full-time basis for anyone other than current employer
  • This position requires the use of a personal vehicle for company use. The candidate must have a valid driver’s licence and maintain $2M Vehicle Liability insurance on the vehicle being used.
