Hospice Social Worker

4-day per week (30 hour) position

Salary Range: $37.00 - $45.00 per hour (commensurate with experience)

Position Overview:

In conjunction with the Interdisciplinary Team, the Social Worker participates in assessment, development of care plan, psychosocial treatment and discharge planning in both in-patient and home care settings for hospice patients. Our organization is dedicated to providing compassionate care to those in need, and we're looking for a skilled and compassionate professional to join our team.

Duties & Responsibilities:

Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of the plan of care.

Conducts ongoing reassessments of patient/family needs and counseling as required.

Provides short term crisis intervention and individual or family counseling when indicated.

Observes, evaluates, and brings to the team conferences information regarding psychosocial, spiritual, physical and financial conditions affecting the patient and family.

Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.

Documents comprehensive psychosocial information clearly and concisely in a timely manner

Active participant in biweekly IDT Meetings

Participates in clinical supervision as required by State law.

Qualifications:

A Bachelor's or Master's Degree in Social Work, Psychology or Counseling required

Massachusetts State Licensure at LSW, LCSW or LICSW level required

Hospice Social Work experience preferred

Strong background in mental health/counseling and experience in health services delivery. 

Ability to conduct assessment of patient/family psychosocial history.
