OVERVIEW: The Transitional Youth Worker provides one-on-one supports to youth who are living in or transitioning out of Memengwaa transitional housing units. The key objective of this role is to support youth to build their personal capacity, in preparation for the next stages of their lives.
KEY RESPONSIBILITES:
· Provide individualized, one-on-one services to Indigenous youth in and from care.
· Alongside staff team, screen referrals and complete intakes to the program.
· Utilizing program tools, complete SMART goal setting and life skill assessments with tenants; follow up on goals and assessments on a regular basis.
· Conduct weekly check ins with youth to implement, review and update goals.
· Refer to and liaise with appropriate resources; advocate for youth when required.
· Collaborate with youth supports to develop and provide comprehensive supports based on identified needs.
· Provide written reports to and work closely with SECFS case workers as required.
· Maintain youth files by completing file logs, keeping copies of critical information, and filing information to the highest standards of confidentiality.
· Attend goal setting meetings with youth and their supports.
· Alongside the tenant services coordinator and/or support workers, conduct suite checks to ensure health and safety requirements are met.
· Teach youth budgeting skills and arrange financial management programming.
· When youth move out of the program, provide ongoing supports through check in and follow up.
· Intervene in crises in a calm and effective manner, including supporting youth to access appropriate and immediate resources.
· Collaborate with other Memengwaa staff to develop programming calendars.
· Facilitate and/or coordinate programming for participants in the Memengwaa program; document attendance and complete workshop reports.
· Recommend support staff schedules, in collaboration with direct supervisor.
· Participate in team meetings & attend training as required.
· Participate in on-call rotation as required.
QUALIFICATIONS:
· Completion of post-secondary training in human services field; combination of education, previous work experience, lived experience and training may be considered.
· Must have a clear Child Abuse, Adult Abuse and Criminal record checks, and prior contact check is required.
· Valid MB class 5 Drivers license and access to a vehicle is required: acceptable Drivers Abstract required.
· Understanding of how systems such as EIA, CFS, CLDS and others function; ability to support youth in navigating systems.
· ASIST, NVCI, First Aid CPR, and other related trainings considered an asset
· Ability to frequently walk multiple flights of stairs may be required.
· Demonstrate knowledge and/or understanding of Indigenous perspectives, First Nations culture and traditional philosophies.
· Ability to work with addictions, mental illness and trauma from a strengths-based perspective.
· Strong knowledge of Community resources.
· Strong interpersonal skills and ability to develop effective working relationships
· Experience with child welfare and/or youth aging out of care.
· Strong oral and written communication skills with excellent documentation skills
· Excellent time management skills.
· Ability to speak Anishinaabe an asset.
