Estrella Home Health Care, Inc is a licensed, insured Home Health Agency owned and operated by three Registered Nurses with 90+ years of combined experience in delivering compassionate, cost-effective personal care.

Personal Care Provider Essential Functions:

  • Observe and maintain a home environment that ensures the safety and security of the Client.
  • Assist with household chores.
  • Provide companionship including social interaction, conversation, emotional reassurance, encouragement, reading, writing, and activities that stimulate the mind.
  • Assist with activities of daily living, personal care, and any other assignments included in the service plan.
  • Perform or assist Client with personal hygiene, including bathing, hair care, mouth care, and dressing when skilled care, skilled transfer or skilled dressing is not required.
  • Performs unskilled and preventative skincare when the skin is unbroken and no chronic skin problems are active.
  • May shave the patient with an electric razor when skilled skincare is not required.
  • Assist Client with unskilled positioning, transfers, ambulation, and exercises when the
  • The client can direct the personal care staff, has sufficient balance and strength to assist.
  • Performs or assists Client with unskilled bowel/bladder care including use of a bedpan, and perineal care.
  • May empty urinary catheter when there is no disruption of the closed system.
  • Reminds the client to take medications that are normally self-administered, or which have been prepared in advance by the Client, family member, and nurse.
  • Assists with nourishment and fluid needs including preparing meals, feeding clients, and offering fluids.
  • Performs household tasks related to Client’s medical needs, including bed linen changes, client’s laundry. Light housekeeping where the Client spends most of his/her time.
  • Provides protective oversight as needed.
  • Observes and reports to the DCS any changes from normal.
  • Assists household management duties.
  • Maintains timely records of home visits including services provided, date and time in and out.
  • Maintains confidentiality in all aspects of job performance.
  • Performs other related functions as required.

Personal Care Provider Knowledge, Skills & Abilities Required:

  • Knowledge of principles and procedures of safety practices in the home care setting.
  • Understanding of family interactions and the aging process.
  • Beginning knowledge of nutrition and foodservice.
  • Ability to observe, report and record patient’s care and condition accurately.
  • Ability to communicate effectively, verbally and in writing.
  • Ability to establish and maintain effective working relationships with patients and families, staff members, and the general public.
  • Performs other related functions as directed.
  • Maintains confidentiality in all aspects of job performance.
  • Complies with all applicable laws, regulations, and standards in the performance of job functions.

Personal Care Provider Working Conditions & Physical Effort:

  • Work is normally performed in a residence or other living facilities.
  • Able to travel from assignment to assignment within the geographic area served.
  • Able to provide proof of valid auto liability insurance if assignment(s) include driving own vehicle to transport the patient.
  • Able to constantly stand, bend, stoop, twist, stretch, squat, kneel, walk, climb stairs and reach freely.
  • Ability to visually and auditorily observe the client.

Personal Care Provider Minimum Education & Experience Requirements

  • Completion of the appropriate twenty-hour training program or successful completion of skills validation via direct observation of the performance of tasks. Home health experience preferred, but not required.
  • A minimum of 6 months of personal care experience is preferred.
  • Must have a current criminal clearance that satisfies Agency, local, state and federal statutes.
  • Able to travel from assignment to assignment within the geographic area served.
  • Able to provide proof of valid auto liability insurance if assignment(s) include driving own vehicle to transport the patient.
