Senior Case Manager – Residential Veterans Program

Use your case management experience to support veterans as they work toward recovery, stability, and greater independence. At Community Hope, you’ll help veterans connect with resources, build supportive relationships, and plan their next steps while contributing to the daily operation of a residential program.
SCHEDULE
• Monday: 2:00–10:00 PM
• Tuesday through Friday: 8:00 AM–4:00 PM

YOUR ROLE
- Provide case management, assessments, recovery planning, advocacy, and connections to community services.
- Help veterans transition into the residence and prepare for discharge or placement in other housing.
- Facilitate recovery support groups, recreational activities, and monthly veteran meetings.
- Provide medication education and conduct on-site drug and alcohol screenings when needed.
- Assist with staff scheduling, program coverage, and emergency shift coverage.
- Help maintain a safe, clean residence and support veterans with housekeeping and facility upkeep.
- Maintain accurate, timely service records and follow agency fiscal and billing procedures.
- Transport veterans in agency vehicles when needed and work with your team to cover shifts and program responsibilities.
QUALIFICATIONS
- Bachelor’s Degree in Social Work, Psychology or a related field.
- Minimum 1 year of case management experience.
- A valid driver’s license in good standing.
