Joint Planning Team - Youth Support Partner

Summary of Responsibilities:

 

Under the general direction of the Joint Planning Team Coach/Facilitator, this person provides services to youth and families who meet the criteria for the Joint Planning Committee, including participating in the Joint Planning Team as an additional advocate for the youth or family to support the development of the Individual Care Plan. In addition, this person is involved in the evaluation activities required to ensure that the Joint Planning Team activities are done in accordance with the principles and practices of the Youth and Family Training Institute. Responsibilities are carried out in accordance with all existing policies, procedures, and regulations, as well as with the mission and values of the company.

 

Essential Functions:

 

Gather prior treatment information

Request and/or complete authorizations for services

Assist families in completing and updating service plans

Assist families in completing referrals

Provide support to consumers, including crisis intervention

Participate in interagency planning

Attend court hearings

Complete written reports

Maintain accurate and up-to-date files

Complete case notes accurately and timely

Meet billing standards once funding by Medical Assistance is established

Complete release of information processes

Participate in recorded trainings as needed

Complete required High Fidelity Wraparound training, as well as annual trainings

 Achieve and maintain state-wide Youth and Family Training Institute High Fidelity Wraparound credential

 Attend weekly supervision/coaching for a minimum of 3-hours per week until credentialed

 Participate in discharge/transition planning

 Complete and document engagement efforts

 Prepare files for closure and storage

 Complete registration and monitoring forms

 Provide on-call coverage

 Assist in completing outcomes data

 Provides oral and written translation (if applicable)

Minimum Requirements (education, experience, etc.)

 

  High School diploma, or GED

  Previous personal experience in High Fidelity Wraparound because of behavioral or mental health issues, preferred

  Life experiences that allow the formation of relationships with youth or young adults that are understanding, patient, culturally aware and respectful

  Willingness to complete High Fidelity Wraparound credentialing, and successful completion within one year of hire

  Under the age of 25 when beginning the High-Fidelity Wraparound Intensive training

 

Required Licenses, Certificates or Knowledge:

 

  Proficient with Windows operating system

 

 Minimum Employment Requirements:

 

  Pre-employment drug screening clearance

  Valid Pennsylvania Driver’s License

  Acceptable Department of Transportation record check

  Reliable transportation

  Act 33, Act 34, and Act 73 clearances

  Immigration and Naturalization (I-9) clearance

 

Working Conditions:

 

  In-office work performing concurrent varied tasks

  Daily travel throughout York/Adams Counties to providers and other human service agencies as well as occasional travel to statewide meetings

 

Safety Hazards of the Job:

 

 Working hours spent predominately in the community. Must, therefore, have awareness of personal/community safety.  There may be a rare occasion during which it is necessary to have physical interaction with an aggressive consumer.
