Residential Services Coordinator

PLEA Community Services is currently seeking a Casual Temporary Full-Time Residential Services Coordinator for our U-Turn/U-Link/Next Step team. The proposed start date for this position is as soon as possible and it will end upon the return of the incumbent. This position requires union membership.

The youth who access these services have not experienced success in other residential settings; they have complex care needs that require supported family care living environments with a heightened level of support and supervision, and a range of specialized services. These youth have been exposed to risk-provoking environments, and present with a variety of disabilities, mental health issues, challenging behaviors, and involvement in the criminal justice system. The incumbent coordinates and monitors all services provided to participants assigned to their caseload, including ensuring that their service plans meet their individual strengths, needs, preferences and circumstances; are consistent with the program's mandate, and are reviewed and updated on a regular basis. The incumbent provides support and direction to Family Caregivers to ensure that they follow the agreed upon service plans, receive the practical assistance they require, understand the services they have agreed to provide, and comply with the agency's policies and procedures.

Key Duties and Responsibilities

  1. Coordinates the arrangements required for the intake and placement of individuals assigned to their caseload, including (a) making sure that all members of the program team have the information they need to welcome the individual, (b) providing the individuals with an orientation to the program, (c) conducting assessments of their individual needs and circumstances, (d) making appointments and referrals to other service providers, and (e) completing all required documentation.
  2. Provides support and practical assistance to all members of the team to ensure all services and support provided to individuals assigned to their caseload is consistent with the mandate, objectives, outcomes, philosophical approach, principles and mandate of the program.
  3. Coordinates the development, implementation, and review of service plans, monthly reports and safety plans in a manner that addresses each individual's circumstances and needs, builds on their strengths and abilities, and identifies the goals and expected outcomes of their participation in the program.
  4. Leads case planning and management for each individual, provides ongoing consultation and advice to the team, and ensures that service plans are regularly reviewed and updated.
  5. Provides orientation to new Family Caregivers and their respite regarding the mandate and specific expectations of the individual(s) placed in their home, the strengths, needs, and circumstances that individual, their roles and responsibilities in providing services and supports to that individual, and the roles and responsibilities of other team members in doing so.
  6. Develops strong collaborative relationships with Family Caregivers and provides them with ongoing coaching including emotional support. Provides information, strategies, and tools they need to ensure the quality of services they have agreed to. Provides the practical assistance required to meet the day-to-day needs and best interests of the person placed in their home; and direction on how to follow the agreed upon service plans.
  7. Develops the transition/discharge plan for individuals assigned to their caseload; Provides support and assistance to persons served and their Family Caregivers to deliver this plan. Supports the person served toward integration and normative functioning in the community, and connects them to the services and resources available in their home communities when they leave.
  8. Facilitates support for the families/caregivers of persons served during the program and after they transition from our service by ensuring appropriate family connections are made when permitted.
  9. Assists Family Caregivers and other members of the team to access available resources in the community that will enable and encourage the person served to develop the necessary educational, personal, social, vocational and self-advocacy skills; and support their cultural identities and traditions; Monitors involvement in such community resources as an integral part of the service plans.
  10. In conjunction with other PLEA training initiatives plans and encourages ongoing opportunities for Family Caregivers to participate in activities that will enhance their knowledge, skills, and abilities; including completing the fundamental quality improvement initiatives applicable to all Family Caregivers.
  11. Organizes and facilitates regular Family Caregiver meetings, which promote information sharing and mutual support, sharing best practices, improve service delivery, ensure the health and safety issues o the participant and care team, provide updates, educational opportunities and leading the discussion on the learning modules. The Residential Service Coordinator ensures the Family Caregiver understands the areas noted above.
  12. Monitors the compliance of Family Caregivers with health and safety standards, their obligations as stated in their contract, and the agency's policies and procedures; completes Quality Assurance checklist on a quarterly basis and the Quality of Services Reviews at least annually.
  13. Participates in the overall administration of the program, with lead responsibility for ensuring that the Care CAMS files of the Family Caregivers and their respite contractors are maintained in a systematic manner and that all records are current, accurate and complete.
  14. Responds to critical incidents and emergencies in current placements. Recognizes potential crises, analyzes such situations accurately, implements strategies to resolve them, and informs the manager when such situations arise.
  15. Establishes strong connections to the community, including maintaining close working relationships with a variety of resources, and formal and informal service providers. Liaises with funders, other agencies, professionals, government officials, representing PLEA in a professional manner.
  16. Maintains necessary participant and program reports and statistics. Ensures that all records, reports, and documentation pertaining to service delivery are accurate, complete and in compliance with PLEA's policies and procedures.
  17. Adheres to all policies and procedures established by PLEA, and meets the performance expectations of the position in a manner that demonstrates compassion and respect for all. Exercises the responsibilities of the position in a manner consistent with the philosophical approach and mandate of the agency.
  18. Takes initiative to uncover underlying participant and Family Caregiver issues and needs. Takes proactive action to resolve issues and needs in a timely manner, including identifying and effectively communicating with those involved or informed. Updates the Program Manager.
  19. Performs other related duties as required and/or as directed by the Program Manager or Program Director.

Required Knowledge, Skills and Abilities

  1. Excellent interpersonal communication, counseling, facilitation, and conflict management skills, including a demonstrated ability to prepare accurate and comprehensive written reports.
  2. Demonstrated proficiency in facilitating other's learning using diverse approaches and the ability to work collaboratively with youth, families/caregivers and other service providers in the community.
  3. Demonstrated ability to develop strong collaborative relationships, work effectively as part of a team, use good judgment, manage conflicts constructively, work with a high degree of independence and be accountable for results.
  4. Excellent organizational and time management skills; demonstrated proficiency in planning work and organizing its completion, establishing priorities and dealing with priority items first, working under pressure, coping with distractions, adapting to changing circumstances, and managing several initiatives concurrently. Ability to deal with regular, routine tasks with limited supervision.
  5. Ability to lead others with the capacity to reflect, promote, and protect the culture and integrity of the agency and the services it provides.
  6. Ability to analyze and solve problems, make decision, and be accountable and responsible for the results. Ability to involve, consult and follow-up with others as required.
  7. Demonstrated ability to develop supportive relationships with a range of individuals, and to be sensitive and understanding of their social realities including issues of race, culture, religion, gender identity, sexual orientation, poverty and lifestyle.
  8. Sound knowledge of government legislation, regulations and accreditation standards. Sound knowledge of social, justice and health service systems and standards, the theory, principles and practices in the field, and with respect to working with vulnerable youth and adults who have need; and their families/caregivers.

Qualifications, Education and Experience

  1. A master's level university degree in the social sciences or human services or a related discipline is required.
  2. At least two (2) years of recent experience working in programs with vulnerable youth or adults, and their families/caregivers, preferably in residential, community care, licensed or foster home setting.
  3. Demonstrated proficiency in Microsoft Windows, Microsoft Office, and familiarity with database programs.


  1. Satisfactory Criminal Record Clearance.
  2. Must possess a valid Occupational First Aid Level 1 Certification or be willing to obtain one prior to hire.
  3. Use of personal vehicle. This position requires travel in the Lower Mainland and the Fraser Valley.
  4. A valid B.C. Driver's License (Class 5 or Class 7), and a suitable driver's abstract.
  5. The usual hours of work in this position are based on a Monday to Friday flextime scheduling arrangement of 35 working hours per week, including daytime and evening hours as required.
  6. Documentation that the applicant is legally approved to work in Canada.
