Case Manager



The Case Manager is responsible for facilitating job training workshops, providing assignments, and assisting participants in obtaining long-term employment and achieving personal goals. S/he is responsible for guiding members in career mapping and creating plans for goals achievement in order to advance the mission of Living Classrooms Foundation. In general, this includes assistance with the classroom training, employment process, provision of job retention services, and cultivation of relationships, both with members and potential employers. The Case Manager reports directly to The James C. Dent House Director.


Primary Responsibilities

·         Assists in the recruitment of potential clients

·         Complete initial intake

·         Facilitate job training workshops, by teaching essential transferrable soft skills for the workplace.

·         Develops with client an Individual Career Plan (ICP) with short-term and long-term goals.

·         Monitors client’s progress towards goal

·         Evaluates and adjust career plans as needed

·         Refers clients to appropriate resources to assist with meeting goals.

·         Maintain client files-including weekly case notes and updating ICP

·         Collects and maintain data necessary to meet funding requirements and statistical reports

·         Maintain program database

·         Lead efforts to establish relationships with employers and assist with job placements of program participants.

·         Prepare participants for an organized job search, including the securing of proper identification, creating appropriate employment documents, and utilizing time management tools.

·         Train participants on interview techniques to help them successful earn employment.

·         Submit reporting data every month on interactions with participants and progress towards goals.

·         Track all program participants progress for one year after program completion.

·         Develop and maintain relationships with partnering companies and agencies.

·         Other duties as assigned


·         Bachelor’s Degree

·         Minimum two years’ experience in case management or counseling

·         Minimum one-year experience in facilitation or teaching

·         Excellent verbal and written communication skills required

·         Ability to foster a cooperative work environment

·         Ability to establish and maintain effective working relationships.

·         Computer skills required
