The RN Case Manager plays a vital role in ensuring the health, safety, and well-being of home care patients through comprehensive assessments, individualized care planning, ongoing monitoring, and staff supervision. Required Skills & Abilities

• Strong knowledge of adult clinical care and home care regulations

• Proficiency with Windows-based systems and healthcare software

• Excellent planning, time management, and ability to work independently

• Strong written and verbal communication skills (English proficiency required)

• Professional demeanor with integrity, flexibility, and confidentiality

• Ability to manage multiple tasks, prioritize effectively, and problem-solve

Education & Experience

• Current Registered Nurse (RN) license in the State of Minnesota

• Two years of experience in home health, med-surg, or acute care preferred
