Job Title: Home Health RN

Location: Baltimore City, MD

Hours: Full time 40 hours per week

Candidates Needed: 2

Pay: $50-$55/hour

Type of Hire: 13-week contract (potential to extend)

Start Date: ASAP

 

Summary of Position:  Performs initial and ongoing patient assessments. Provides professional, comprehensive, collaborative family-oriented nursing care to individuals in the home setting. Assesses patient’s and family’s needs and formulates, implements and evaluates plan of care. Responsible for providing professional nursing care for patients following established nursing standards and practices. Functions as a clinical liaison between patients and physicians. Provides supervision and direction to the License Practical Nurse and Home Health Aide providing care in the Medicare certified home care program. This position reports to the Clinical Manager. No direct reports.

 

Essential Functions:

 

·         Collects health status data on a patient relevant to his/her presenting problem in a continuous systematic and complete manner. Utilizing available resources, patient family/significant others and health care records in the collection of health status data

·         Completes thorough and accurate OASIS documentation

·         Document and complete plan of care assessment and visits in a timely, accurate, concise, legible, and appropriate manner as per agency policy

·         Throughout patients stay, assesses teaching/learning needs of patient/significant others while preparing for discharge

·         Makes referral to appropriate community resources and completes episodes in timely manner.

·         Documents patient and caregivers understanding of information

·         Formulates, implements, coordinates, and educates care in a knowledgeable, skillful, consistent, and safe manner

·         Collaborates with other clinicians, family, and caregivers in order to maximize positive outcomes.

·         Develops therapeutic nursing interventions to meet patient’s individual goals

·         Evaluates, documents, and revises diagnosis, outcomes, and interventions based on evaluations of patient outcomes and progress towards goals

·         Demonstrates sound knowledge and skills in the care and decision-making of age-specific patient populations: adult and geriatrics

·         Responds timely to requests for additional information, corrections or orders from QAPI or finance departments

·         Read k-mails daily and respond as needed in a timely manner

·         Participates in all case conferences and mandatory meetings

·         Demonstrates team-building skills and serves as positive role model/resource for other team members

·         Assumes role of Case Manager and provide episode management per agency policy

·         Participates in cost containment measures including utilizing supplies efficiently, accurately recording all charges, executing efficient patient flow, utilizing work time productively, and identifying actual potential problems

·         Participates in the orientation of new Associates as assigned

·         Ongoing interdisciplinary assessment of the patient

·         Participate in interdisciplinary meetings per regulations

·         Development and evaluation of the plan of care in partnership with the patient, representative (if any) and caregivers

·         Providing services that are ordered by the physician as indicated on the plan of care

·         Patient, caregiver and family counseling

·         Patient and caregiver education

·         Prepare complete and accurate clinical notes

·         Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care

·         Monitor episodic utilization and authorization management

·         Onsite supervision of care provided by Home Health Aides (HHA) and instructs and/or reviews HHA services no less frequent than every fourteen (14) days.

·         Modifies Patient/HHA Care Plan at appropriate intervals and advises HHA of modification and documents appropriately

·         Provides supervision to Licensed Practical Nurses as dictated by agency policy and state practice guidelines, not less frequent than every thirty (30) day

·         Appropriately and efficiently utilizes proper channels of communication with colleagues, physicians, clients, and caregivers, to ensure clear concise flow of patient information and services

·         Maintain a safe working environment through adhering to client’s policies and procedures in the office and patients’ homes.

·         Represents client in the community and participates in community liaison activities

 

Supervisory Responsibility:

 

·         Provides supervision to Licensed Practical Nurses and Home Health Aides per policy/regulations

 

Education/Experience:

 

·         Current Registered Nurse licensure in the state of Maryland

·         Current American Heart Association BLS CPR for healthcare provider certification

·         Graduate of an accredited school of nursing

·         Minimum of two (2) years community health nursing experience

·         Minimum of three (3) years acute care setting experience required

·         Good understanding of OASIS and clinical documentation of assessments

·         Fleet safety requirement and a valid driver’s license and auto insurance – travel required

·         Knowledge of electronic medical records, Kinnser experience preferred

·         Basic knowledge of Medicare/Medicaid/Commercial Insurance

 

Communication Skills and Abilities:

 

·         Excellent written/verbal communication skills

·         Electronic medical record documentation and reporting system skills

·         Microsoft Office knowledge

·         Communicate with the QAPI department and complete all requests timely

·         Participation in sponsored in-service training
