role of the LPN Team Leader is coordinate and assist with the provision of
Nursing Care to Residents and to oversee the delivery of care by assigned
Personal Care Assistants in accordance to the policies and procedures along
with the Mission and Core Values of Fellowship Community and DHS regulations.
The LPN-Team Leader functions under the
direction of the Director of Resident Care and the nursing supervisor.
ESSENTIAL QUALIFICATIONS / CREDENTIALS
have graduated from licensed nursing school with an active PA LPN license
have knowledge of DHS regulations related to Personal Care.
have certificate in Basic Life Support.
have computer skills at an intermediate level.
to read, write, speak and comprehend English.
of the special needs and care requirements of geriatric residents.
to effectively present information in one-on-one and small group situations to
customers and other employees of the organization.
well with others in a positive and cooperative manner.
to be flexible to meet the needs of the organization.
Care Home Medication Training, CPR and First Aide qualified
demonstrate effective leadership skills in delivery and coordination of
performance reflects the essential elements of Fellowship Community’s Core
ESSENTIAL DUTIES AND RESPONSIBILITIES OF RESIDENT CARE
consult with residents, families, and physicians in providing resident care,
treatment, rehabilitation and discharge.
adequately complete correct and comprehensive resident forms as required by DHS
Regulations and under the direction of the Director of Resident Care. To
include, but not limited to the following: Med Evaluations, Support plans,
Resident Assessments, fall risk assessments, Care Plans, etc.
injections such as IM meds. Or any subsequent medications not in the PCA Team Leader’s
guidelines set forth by DHS med. administration classifications.
and directs the delivery of care by the PCA team members designated to their
input into the performance reviews of the PCA team members.
dietary staff at resident meals and report any changes in resident’s meal
completion to physician/Director of Resident Care/nursing supervisor.
report at the beginning of shift as well as giving report at the end of shift
inclusive of narcotic count and ensuring accuracy of count.
residents chart for specific treatment, medication orders, diets and review
proper order of charts.
with physicians for medical and/or prescription needs of residents in the
Terrace and write appropriate orders and proper transcription of same.
any changes of residents medical, psychological or mobility needs to the
Director of Resident Care, Clinical Care Supervisor or the Physician when
as the Administrator Designee as directed by the Administrator, Director of
Resident Care or Nursing Supervisor.
