Reporting to the Street Level Supervisor, the Street Level Community Outreach Worker is a member of a multidisciplinary outreach team that aims to meet the social determinants of health for individuals experiencing multiple barriers which can result in chronic homelessness and/or increased utilization of emergency health services. The Street Level Community Outreach Worker will be cross-trained in both advocacy services as well as crisis intervention techniques in order to contribute to a mobile outreach program that provides addictions services to vulnerable clients on the verge of or experiencing homelessness and who are not able to access conventional services. The Street Level Community Outreach Worker will act as a first responder in moments of potential crisis and will also use utilize their knowledge of advocacy resources in order to serve community members and connect them to appropriate services. The Street Level Community Outreach Worker will also serve as a connection for community members, bridging the gap from outreach services to The Mustard Seed’s established downtown services. Additionally, they will work in collaboration with external community agencies and build partnerships with them so that community members are supported to the fullest extent. 

Position Type: Full time, Fixed term until May 7, 2022
Schedule: Monday to Friday, 8:30 am to 5:00 pm

DUTIES AND RESPONSIBILITIES

  • Provide care for clients in need via food, essential items, and referral to services
  • Evaluate and assess needs of community members to determine which services would most appropriately support them
  • Through consistent and courteous interaction, build relationship and trust with various community members in order to better connect them to resources
  • Help facilitate the clients’ optimized utilization of all available community supports as well as their transition to those community supports – this could include but may not be limited to coordinating and transporting clients to their appointments
  • Support co-workers and work collaboratively within the multidisciplinary team to provide comprehensive case management to clients as needed
  • Document client interactions, client referrals, and client progress on the Homeless Management Information System (HMIS)
  • Facilitate emergency response and crisis intervention, as needed 
  • Develop and maintain effective working relationships within The Mustard Seed and with external agencies
  • When not involved in outreach, provide services at downtown Street Level location as needed
  • Track statistics and prepare reports, as required
  • Assist Addictions Mental Health Outreach Worker and the Street Level Supervisor with new initiatives or projects as required
  • Other duties as assigned

MINIMUM QUALIFICATIONS AND SKILLS

  • A passion to be part of ending homelessness in Alberta
  • Bachelors Degree in Behavioural Science or Social Work is an asset
  • Experience working in Mental Health is an asset
  • Experience working with vulnerable populations and individuals experiencing homelessness 
  • Ability to effectively communicate, both verbally and in writing, with clients, members of the multidisciplinary team, and community agencies
  • Excellent interpersonal skills that lend themselves to effective crisis management as well as forming sustainable connections with community members
  • Knowledge of Calgary’s community mental health and addiction resources as well as available support services for minority groups 
  • Ongoing evaluation and assessment of client’s needs in order to properly refer them to the most appropriate supports
  • Ability to problem solve and adapt in the midst of adversity
  • Knowledge of FOIP and ability to observe confidentiality standards
  • A self-starter with the ability to work in a cross-functional team and accomplish goals
  • Proficient computer skills (Microsoft Office Programs); experience with Homeless Management Information System (HMIS) an asset
  • Elevated organizational, time management, and advocacy skills
  • Valid Driver’s License (Class 5) and access to a reliable vehicle

WORKING CONDITIONS

  • This position is a mixture between mobile street outreach and working out of a Support Center with the existing Street Level team downtown
  • Lifting, travelling, sitting, standing, walking, and driving personal vehicles are required as part of this role
  • Moderate physical effort required to successfully accomplish the essential job functions, such as responding to a fast-paced work environment and entering data into databases

  • Must have legal authorization to work in Canada on a full-time basis for anyone other than current employer.
  • This position requires the use of a company vehicle. Candidate must have and maintain valid driver’s licence and will need to consent to a driver’s abstract request.
