Gail’s Home Care is seeking a compassionate and qualified Registered Nurse (RN) to conduct initial assessment visit for our clients in Crawfordville, GA 30631.

The RN will be responsible for conducting an initial assessment of the client and assisting with the development and implementation of an appropriate service plan based on the client’s needs.

• Graduate of an accredited school of nursing

• Valid and current Registered Nurse (RN) license

• Current CPR certification

• Previous home care or assessment experience is a plus

• Compassionate, dependable, and professional

• Ability to work independently and communicate effectively with clients, families, and care team members

• Conduct an initial assessment of the client’s needs and condition.

• Develop and implement the service plan as applicable.

• Regularly reassess the client’s needs and recommend changes when appropriate.

• Coordinate services to support the client’s care needs.

• Observe and report changes in the client’s physical or emotional condition.
