Physical Therapist (PT) Responsibilities

As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services designed to maximize functional independence and support successful rehabilitation outcomes.

Responsibilities include:

  • Evaluate residents' mobility, strength, balance, range of motion, functional abilities, and rehabilitation needs.
  • Develop and implement individualized physical therapy treatment plans based on resident goals, clinical findings, and physician orders.
  • Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, coordination, and overall functional independence.
  • Treat residents recovering from a variety of conditions, including orthopedic injuries and surgeries, neurological events such as stroke, cardiopulmonary conditions, multiple traumas, and other complex medical conditions.
  • Monitor resident progress and modify treatment plans as clinically appropriate.
  • Exercise sound clinical judgment and respond appropriately to changes in resident medical or functional status.
  • Educate residents and families regarding treatment plans, therapeutic exercises, mobility techniques, and strategies to promote safety and independence.
  • Recommend and assist with appropriate adaptive equipment and mobility devices.
  • Maintain accurate, timely, and complete clinical documentation in accordance with regulatory and facility requirements.
  • Collaborate with physicians, nursing, occupational therapy, speech therapy, and other members of the interdisciplinary care team.
  • Participate in interdisciplinary care conferences and family meetings as appropriate.
  • Promote resident safety, dignity, independence, and quality of life throughout the rehabilitation process.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current and valid Pennsylvania Physical Therapist (PT) license required.
  • Previous experience in skilled nursing, long-term care, rehabilitation, acute care, or a related healthcare setting preferred.
  • Strong clinical assessment, critical-thinking, and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
  • Commitment to providing compassionate, resident-centered rehabilitation services.
