Social Services Coordinator
The Social Services Coordinator will work with residents in the healthcare center by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided with strict adherence to government regulations and CH Policies. This position manages both short term rehab and long term care residents. Majority of the role is discharge planning for the short term rehab patients, including assistance with placement as needed, referrals to home care or hospice and coordinating the ordering of durable medical equipment. This role provides education to families and patients as they navigate insurances and the transition to home or other placement. In addition, this position schedules and facilitates care conferences.

Required Attributes:
Education: A Bachelor of Science degree in social or human services.
Experience: Minimum of 1 year of experience as a social worker. Prior work with elders in a community or long-term care setting is preferred.
Skills: Good organizational skills, attention to detail and ability to multitask in a fast paced environment. The Social Services Coordinator must have skills in communication, assessment and social work methods and techniques. He/she should be able to work effectively with a variety of disciplines in an individual and team setting. He/she should have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.
