KNOWLEDGE/EXP: High School Diploma, Proficient in
Microsoft Excel, Word, and PowerPoint.
2+ Years in Mental Health or a Medical Office and must have good phone
skills, organizational skills, and attention to detail.
SUMMARY: Assists Care Coordination department staff
with coordination of aftercare appointments and making initial contact with
parents and guardians, medication refills, school requests, and food bank
assistance. Assists the Director with reporting and auditing requirements and
maintains community and PPR data registry.
