That’s why we are looking for a Case Manager who can use discretion and independent judgment, and who is responsible for coordinating the delivery of quality patient care by assuring that the nursing care plan is followed.
Essential Duties & Responsibilities:
Plans for the patient’s discharge.
Ensures that all disciplines are scheduled based on plan of care and order frequencies and that visits are made timely.
Evaluates the effectiveness and outcome of care.
Provides for activities of daily living as needed
Maintains a safe patient environment
Responsible for creating and supporting a positive, professional, team-oriented, harassment-free work environment by understanding and complying with the Company’s policies, by demonstrating the Company’s competencies, and being a role model for the Company “HEARTBEAT” Values.
Education, Experience, Skills, & Abilities:

High school diploma required. College degree preferred
Some proofreading and editing skills.
Flexibility with schedule, including availability to work evenings and weekends
Team player who is motivated and has excellent communication skills
Must enjoy working with a diverse population
Residential Case management experience preferred
