PLEA Community Services is currently seeking two (2) Regular Full-Time Community Inclusion Workers for our FolkStone teams. The proposed start date is March 15, 2020. This position requires union membership.

The residents who access the Folkstone program are adults who have complex personal and social needs that require a heightened level of support, health care, medical supervision, and specialized services. The incumbent is responsible for assisting residents to participate in educational, vocational, social, recreational and cultural activities that are available in their communities – and to achieve their individual goals, as identified in their service and nursing plans.

Key Duties and Responsibilities

  1. Provides residents with opportunities to access available resources in the community by supporting their participation in social, recreational, cultural, educational and vocational activities.
  2. Assists residents to function more independently in the community by encouraging them to develop the necessary personal, social and life skills, and to achieve their individual goals, as identified in their service and nursing plans.
  3. Drives and/or accompanies residents to appointments, meetings, and other activities, as directed.
  4. Maintains on-going positive relationships with Family Caregivers and participates as part of the team responsible for enhancing the quality of life for residents and addressing their individual strengths, needs, abilities, and preferences consistently.
  5. Develops collaborative relationships with residents’ families/caregivers, significant others and informal support systems to maintain and strengthen these connections.
  6. Engages the residents in acceptable recreational and social activities in the community that will encourage new interests, strengthen their resilience and develops their interpersonal communication skills.  Teaches life skills and community survival skills, provides emotional support, and models acceptable behavior.
  7. Adheres to all policies and procedures established by PLEA, and meets the performance expectations of the position in a manner that demonstrates compassion and respect for all persons served. Exercises the responsibilities of the position in a manner consistent with the philosophical approach and mandate of the program.
  8. Performs other related duties as required.

Job Skills and Abilities

  1. Good interpersonal communication skills, including a demonstrated ability to write accurate notes.
  2. Demonstrated the ability to work with others; enjoys the company of adults and feels comfortable working with those living with developmental disabilities, an acquired brain injury, FASD, dementia, and other mental health issues.
  3. Demonstrated ability to develop supportive relationships with residents and their families/caregivers, and to be sensitive and understanding of their social realities including issues of race, religion, culture, gender identity, sexual orientation, poverty, and lifestyle.
  4. Demonstrated ability to encourage and support adults to participate in community activities, use good judgment, and to follow all instruction and protocols.
  5. Ability to work collaboratively on a team with other program staff, and with others in the community including professionals, collaterals, and families/caregivers, informal supports and community members. Demonstrated ability to work independently and be accountable for results.

Qualifications

  1. At least a two-year diploma from a post-secondary institution in the health or social services field, or an equivalent combination of education, training, and experience.
  2. Recent volunteer or work experience working with individuals who require specialized supports and guidance to participate in the life of their community.
  3. Demonstrated proficiency in Microsoft Windows, Microsoft Office, and familiarity with database programs.

Requirements

  1. Satisfactory Criminal Record Clearance.
  2. Must possess a valid Occupational First Aid Level 1 Certificate or be willing to obtain one prior to hire.
  3. Use of Personal Vehicle. This position requires travel in the Lower Mainland and the Fraser Valley.
  4. A valid B.C. driver's license (Class 5 or Class 7), and suitable driver's abstract. 
  5. The usual hours of work are different for each of these positions and are as follow:
    1. ) Schedule 1: Monday, Wednesday, Thursday and Friday 9:00 AM to 5:00 PM, and Tuesday 12:00 PM to 8:00 PM, based on 37.5 hours per week;
    2. ) Schedule 2:  Tuesday to Saturday 9:00 AM to 5:00 PM, based on 37.5 hours per week.
  6. Documentation that the applicant is legally approved to work in Canada.
