The Program of All-Inclusive Care for the Elderly (PACE) is one of the most rewarding settings in rehabilitation. As a PT in our PACE program, you will build long-term therapeutic relationships with elderly participants, helping them maintain independence and remain in their communities. Unlike acute or post-acute settings, PACE allows you to follow patients over time, see the cumulative impact of your interventions, and truly get to know the people you serve.
What You Will Do
You will provide comprehensive physical therapy evaluations and treatments for PACE participants in a community-based setting. Your work will focus on fall prevention, mobility optimization, pain management, and functional maintenance. You will collaborate with an interdisciplinary team including physicians, nurses, social workers, and occupational therapists to develop holistic care plans. The pace of work is steady and sustainable, with an emphasis on quality over volume.

What You Bring
• DPT or Master's degree from an accredited PT program
• Current or eligible for Pennsylvania State PT license
• Interest in geriatric care and community-based health
• Strong assessment and communication skills
