Position Overview

We are seeking an experienced and compassionate Registered Nurse (RN) to join our hospice team.

This field-based role involves serving patients throughout Philadelphia County and requires strong clinical judgment, independence, and a commitment to delivering high-quality, patient-centered end-of-life care in the home setting.

What You’ll Do

• Conduct comprehensive initial and ongoing patient assessments

• Develop and manage individualized care plans

• Provide skilled nursing care, pain and symptom management

• Educate and support families during end-of-life transitions

• Document visits accurately and in compliance with regulatory standards

• Participate in interdisciplinary meetings and on-call rotation.

What You Bring

• Active Pennsylvania RN license (required)

• Minimum 2 years of hospice case management experience strongly preferred

• Strong clinical assessment and documentation skills

• Ability to manage a caseload independently

• Valid driver’s license and reliable transportation

• Willingness to travel within Philadelphia County
