  • Help members set and reach their own goals, from wellness and coping skills to relapse prevention, employment, and daily living
  • Connect members to housing, healthcare, education, transportation, benefits, and other community resources
  • Take part in Person-Centered Planning and contribute to your members’ plans
  • Use motivational interviewing and other recovery-oriented strategies, with training and support from your Team Lead
  • Document services accurately and on time in our electronic health record
  • Share progress and concerns with your Team Lead and the CST
  • Respond to crises and emergencies using agency procedures, with team backup

What You Bring
Required:

  • Personal lived experience in recovery from a mental health and/or substance use disorder
  • High school diploma or GED
  • Valid NC driver’s license, reliable transportation, and current auto insurance
  • Ability to travel throughout our service area
  • Basic computer and documentation skills
  • Willingness to complete NCI-B and CPR certification

Preferred:
  • Community-based behavioral health experience
  • Familiarity with NC Medicaid behavioral health services
  • Experience with an electronic health record

What the Job Looks Like
  • Mostly community-based: members’ homes, community locations, clinics, and other approved settings
  • Regular hours are Monday-Friday, 8:00 a.m.-5:00 p.m., with some evening or weekend flexibility to fit members’ schedules
  • Time in the office for documentation, team meetings, supervision, and training
  • Regular driving, walking, and standing, and occasional lifting up to 25 lbs

Why Pathways to Life
  • Real team support: supervision from a CST Team Lead and a collaborative team
  • Training that builds your skills, including
  • Room to grow as the program expands
