The Opportunity:

DIVERSEcity is seeking an Intake Coordinator for our Settlement & Community Development team. This position will be responsible for the maintenance, administration, and assessment of all referrals, intakes, and initial assessments and triage of clients.

Key Responsibilities:

  • Coordinates and responds to incoming departmental referrals;
  • Completes intakes and assessments as required;
  • Liaises with clients, prepares case histories and outlines services to be provided by the organization. Refers clients to more appropriate programs if needed;
  • Applying an intersectional approach, will provide insight and recommendations on client referrals and service plans:
  • Maintains current knowledge of departments services, eligibility criteria, wait-list period, and overall service profile;
  • Responsible for development and maintenance of case load lists, attendance sheets, wait-lists, etc., as well maintaining relationship with referring agencies;
  • Liaises with other service providers regarding referrals and program services; 
  • Responsible for recruiting, selecting and registering program participants for groups and other programs based on program eligibility criteria and their needs;
  • Assists with groups as a support when needed;
  • Provides necessary information about department services to service providers, clients, and stake holders, communicating, when possible, in clients first or most proficient language;
  • General program administration including reception, filing, minute taking, room booking, program communication and program record keeping;
  • Promotes the programs through such activities as the distribution of program brochures and posters, local and ethno-culturally-focused media advertising, and making presentations to community groups or organizations;
  • Develops appropriate resources that support the delivery of each program;
  • Ensures the necessary facilities, equipment and materials are available for the programs;
  • Maintains up-to-date knowledge of community resources to provide program-related information to participants;
  • Works closely with other internal programs, staff, and volunteers for the purpose of referral and integrated service delivery;
  • Maintains program records containing statistics, and other paperwork relevant to the program and specifically statistics on referrals and service delivery.
Mandatory Qualifications:

  • Degree or Diploma in social services or related field;
  • Two (2) years' experience in administration, coordination or related experience or an equivalent combination of education, training and experience;
  • Professional level Fluency in English and Punjabi or Arabic.
Qualified candidates will also posses: 
 
  • Experience working with children, youth & families;
  • Experience with trauma-informed and attachment-focused approaches;
  • Cross-cultural training and/or experience in working in a multicultural environment;
  • Ability to work with minimal supervision and meet deadlines;
  • Sound knowledge of community resources and ability to network and build partnerships;
  • Familiarity with outcome-based program evaluation strategies.
