Job Summary: Brockton Home Health care Agency is currently looking for a full time Home Care Office LPN, also known as a Licensed Practical Nurse (LPN). The LPN will be providing administrative and clinical support, rather than direct patient care in the home. This role involves tasks like coordinating care plans, managing paperwork, review (PA) Prior Authorization before submission to insurance providers, review admission packets for accuracy, and communicating with patients and families.
Required Skills:
· Licensed Practical Nurse (LPN) license in Massachusetts.
· Strong knowledge of medical terminology and healthcare practices.
· Excellent communication and interpersonal skills.
· Ability to work independently and as part of a team.
· Experience in a home health setting or office setting is preferred
· Strong communication skills: Both verbal and written, to effectively communicate with insurance providers, clients and caregivers.
· Time management skills: To effectively handle multiple tasks and meet deadlines.
· Computer proficiency: Internet skills, including Microsoft Office suite and Axxess experience
Administrative Tasks:
· Managing referrals, auditing paperwork, and answering questions.
· Care Planning: Collaborating with RNs to develop and modify patient care plans, ensuring they are patient-specific and meet individual needs.
· Communicating with patients, families, and other healthcare professionals about care plans and patient status.
· Maintaining accurate and up-to-date patient records and documenting care provided.
· Supervising and delegating tasks to Home Health Aides (HHAs
· Assisting with medication administration, wound care, and other basic nursing tasks, as needed.
· Current and Valid LPN License:
· Must be licensed as a practical nurse in Massachusetts by the Massachusetts Board of Registration in Nursing.
· CPR Certification:
· Knowledge of nursing principles, practices, and procedures, as well as experience in home health or a similar setting is often preferred.
· Excellent communication, interpersonal, and customer service skills are essential.
· Familiarity with computer systems and electronic health records is often required.
