Licensed Practical Nurse - Homecare

Vitality at Home is seeking a compassionate and drive Licensed Practical Nurse (LPN) to join our Homecare team.  In this role, you will provide high-quality nursing care in clients' homes, supporting their independence while ensuring compliance with all state regulations and clinical standards. 

You will work under the direction of an RN to monitor client conditions, implement care plans, and deliver safe, effective care.

What You Will Do:

• Monitor client conditions and report changes to the RN
• Perform focused assessments and support care plan implementation
• Administer medications and ensure accurate documentation
• Assist with physician orders and care coordination
• Support onboarding and mentoring of new team members
• Respond to client and family concerns with professionalism and urgency
• Participate in quality improvement and organizational initiatives

What We Are Looking For:

• Licensed Practical Nurse (LPN) in the state of Minnesota (active & unencumbered)
• 2+ years of experience in home health, med-surg, or acute care preferred
• Valid driver's license, reliable vehicle, and auto insurance
• Strong communication, organization, and critical thinking skills
• Ability to work independently while maintaining a high level of accountability
