A growing Skilled Nursing Facility in the Melbourne, Florida market is seeking an experienced Hospital Liaison / External Marketer to drive census growth through hospital and healthcare referral relationships.
This role is built for someone who knows the SNF space, understands post-acute referrals, and can actively build and maintain relationships with hospital discharge planners, case managers, and healthcare partners throughout the region.

Responsibilities
- Develop and maintain referral relationships with hospitals, case managers, discharge planners, and healthcare providers
- Increase patient admissions and overall SNF census growth
- Conduct outside marketing visits throughout the assigned territory
- Educate referral sources on facility services, clinical capabilities, and admissions process
- Coordinate closely with admissions and leadership teams
- Track referral activity and maintain strong follow-up communication
- Represent the facility professionally within the local healthcare community
Requirements
- Prior Skilled Nursing Facility (SNF) experience required
- Experience as a Hospital Liaison, Clinical Liaison, Community Liaison, External Marketer, or Healthcare Marketer required
- Existing hospital relationships/referral network strongly preferred
- Strong understanding of post-acute care and SNF admissions process
- Proven relationship-building and business development skills
- Reliable transportation and willingness to travel locally
