SUMMARY:  Family Connects is an evidence-based nurse home visiting program that connects parents of newborns to the community resources they need through postpartum nurse home visits. 

This position oversees all aspects of program delivery and promotion. The Program Manager is responsible for program staff, locations, fiscal management, policy implementation, and the pursuit and maintenance of certification through Family Connects International. This position is responsible for developing and fostering relationships with community organizations, funders and a diversity of stakeholders in order to promote program success and sustainability. Ability to communicate effectively with others is essential. Experience in maternal/child health, home visiting, and/or lactation education preferred. 

RESPONSIBILITIES:

• Work closely with Family Connects International to obtain and maintain program certification. 

Ensure program operations and activities adhere to fidelity guidelines as identified by Family Connects International (FCI). 

Maintain regular communication with the FCI Implementation team. 

Work with Family Connects International staff and site staff to implement process improvement strategies and maintain fidelity to the Family Connects model. 

Manage program staff in order to provide supportive feedback, identify process improvement opportunities and help resolve complex problems. 

Coordinate assignments to the nurses to provide home visiting services

Report program metrics, operations data and pertinent program data to various stakeholders via monthly and quarterly reports.

Coordinate Community Advisory Board process in collaboration with Community and Resource Coordinator staff. 

Assist in development and adherence to program and operating budget.

Develop and maintain partnerships with state and local organizations, as well as other Family Connects program replication sites.  

Support development and execution of growth strategy and scaling plan. 

Develop and maintain communications with funders, seek grant opportunities and assist with program promotion. 

Coordinate staff meetings for case review and in-service trainings.

Participant in inter and cross agency meetings that may include the FCM Advisory Board, care clinics, doctors’ offices and birthing hospitals and/or make calls based on birth records.

Work closely with non-clinical support staff in all program functions, including establishing a parent recruitment protocol and ongoing program implementation.

Act as a liaison between Family Connects and Sinai Hospital to ensure that Family Connects scheduling protocol is followed.

Complete statistical reports as required.

Meet weekly with Director of Programs and Research to discuss cases and problem solve issues with program implementation

Attend required staff meetings, training, consultation meetings and conferences.

Participate in agency work teams as assigned.

EDUCATION:

Bachelor’s degree in a relevant field (e.g., healthcare administration, public policy, public administration, social work, psychology, or public health with quality improvement/systems emphasis). 

Masters degree preferred.

EXPERIENCE AND REQUIREMENTS:

Minimum 3 years of experience in program management. 

Excellent knowledge of data analysis, reporting and budgeting. 

Ability to direct a team of clinical and non-clinical staff. 

Experience working in community-based programs preferred.

Experience and willingness to work with a culturally diverse population.

SKILLS:

Excellent verbal and written communication skills. 

Exceptional organizational and leadership skills. 

Experience with large-scale budgeting and growth strategies. 

Experience in maternal/child health or home visiting preferred. 

Thorough understanding of project/program management techniques and methods. 

Knowledge of community resources and agencies

Ability to build partnerships 

Proficient computer skills including the use of Microsoft Office Suite, DocuSign, and assigned databases. 

WORK ENVIRONMENT:

Must work flexible hours, including weekends and evenings

Must be prepared to travel to various program sites and various communities.

NUMBER OF POSITIONS SUPERVISED: 1 Nurse Supervisor, 2 Coordinator, Community Resource & Support, 3 Home Visiting Nurses
