MDS Nurse

Full-Time MDS Nurse (37.5 hrs/week) at Liberty Commons Skilled Nursing & Rehab in Chatham, Cape Cod

There is a $12,000 sign-on bonus for this position.

Pay Rate: $50/hr+ (based on experience)

Broad Reach Healthcare / Liberty Commons Rehab & Skilled Care is seeking an experienced MDS Nurse to join our interdisciplinary team.
This new full-time (37.5 hours/week) position will expand our MDS department to four nurses, strengthening our ability to manage a high-volume, high-acuity case mix while ensuring compliance, timeliness, and optimal reimbursement capture.

Role Responsibilities:

• Complete case start-to-finish: DX, MDS, Care Planning, Case Management
• Lead and manage managed care caseloads while flexing to support LTC and Medicare MDS as needed
• Conduct timely and accurate assessments (5–6 per week typical in LTC, plus subacute/managed care)
• Collaborate with IDT to ensure compliance with federal/state regulations and QRP thresholds
• Support training initiatives (e.g., Section GG) and contribute to ongoing staff development

Ideal Candidate:

• Licensed RN or LPN with strong clinical assessment skills
• Minimum 1–2 years MDS/RAI experience required
• Strong knowledge of Medicare, Medicaid, and Managed Care processes
• Highly organized with excellent communication and collaboration skills
• Detail-oriented, with a commitment to compliance and quality

• Full-time, 37.5 hours/week
